One platform to make health and community networks operate as one.
Humaximus coordinates your patients, care teams, partners, funders, and resources through one autonomous network where health and community organizations mobilize, execute, and adapt together.
Patients carry the burden. Organizations lack the shared execution layer.
Patients are left navigating steps, costs, eligibility, and follow-up. Organizations are left managing shared work through disconnected systems, unclear ownership, and manual coordination.
Patient reality
I keep repeating my story
I don't know what happens next
I don't know what I qualify for or what it will cost
I have to manage the scheduling and logistics
I still don't know if the care or service happened
Organization reality
We don't have the full information to act
We can't safely share the right information with the right people
We don't know who owns the next step
We can't tell which partner can actually take this
We can't clearly prove the work happened or solved the need
Patients navigate alone. Organizations coordinate blind. Same broken journey.
Who Humaximus is for
Built for the organizations responsible for coordinated care.
Humaximus supports the organizations that need patients, care teams, partners, funders, and resources to move together through one shared execution layer.
Health Organizations
For hospitals, clinics, behavioral health providers, pharmacies, and care teams coordinating patient journeys across settings.
Community Organizations
For housing, food, transportation, benefits, workforce, and other service organizations supporting people beyond the clinical setting.
Conveners & Networks
For United Ways, ACOs, government agencies, coalitions, and backbone organizations aligning many partners around one shared flow of action.
Funders & Resource Partners
For sponsors, foundations, and resource partners that need proof of what support was delivered and what it achieved.
The Platform
One autonomous coordination layer for the entire care network.
Humaximus brings people, care teams, partners, resources, and outcomes into one shared execution system — so every need can move from identification to action, support, proof, and verified outcome.
